IMPORTANT INFO ABOUT MEDICATIONS
To all parents of scouts going to summer camp
If your child needs medication for any reason while on a scouting event you must fill out a medication authorization form. This includes both prescription and over-the-counter medications. This form is always available on the Troop 313 website - Forms & Lists.
For summer camp – The attached form is interactive so you can fill it out on your computer before you print it. Note: Acrobat Reader will allow you to open, fill out, and print the form. Acrobat Pro is needed if you want to save the form for later use. You may still complete the form by hand. Either way is fine. |
Medication Form - INTERACTIVE
THIS AUTHORIZATION INCLUDES OVER-THE-COUNTER MEDICATIONS. If you send your own medication, i.e. Tylenol, Benedryl etc. it must be in the original bottle and must have Scout's name on the bottle.
Please follow these instructions for preparing your child’s medication to take to summer camp.
1. The medication form must be filled out, signed and accompany the medicine.
2. The original container from the pharmacy must accompany all prescription medications.
3. Place both of these in a zip-lock bag with your child’s name on the outside.
- If there are any pills that need to be split – this must be done before you hand over the medications to the Medication Officer for the scouting event.
- If your child has multiple medications that need to be given, please place the pills in a separate zip-lock bag for each dose. For example – one bag for Monday morning, one bag for Monday noon, one bag for Monday evening, etc. If all days are the same – you may simply have 7 morning bags, 7 noon bags and 7 evening bags.
- Please do not put the pills in the traditional “pill boxes” labeled for the days of the week. It is easy for the tops to open and the pills to fall out.
I will be the medication officer for summer camp. If you have ANY questions concerning this whole procedure, please give me a call - (314) 965-2535 home or feel free to call me at work during the day (636) 733-7000.
